Two regular cleanings per year
Two routine exams
One emergency exam annually
X-rays: four bite-wings, two periapical (PA) per year, and a full mouth set every 3-5 years
Two fluoride treatments
Oral cancer screening
Gum disease screening

Additional Benefits:

10% discount on services like whitening, orthodontics, implants, cosmetic dentistry, crowns, and more.

Pricing:

First Member:
$400 annually or $36 monthly ($432/year)

Spouse:
$380 annually or $34 monthly ($408/year)

Each Additional Dependent (up to age 23):
$320 annually or $29 monthly ($348/year)

Perio Plan

includes:

Three periodontal maintenance cleanings per year
Two routine exams
One emergency exam annually
X-rays: four bite-wings, two PA per year, and a full mouth set every three years
Oral cancer screening
Gum disease screening

Additional Benefits:

10% discount on services such as crowns, fillings, root canals, extractions, sealants, and dentures.

Pricing:

Perio Plan Member:
$490 annually or $44 monthly ($528/year)

The Platinum Plan

X-rays: 4 bite-wings, 2 PA’S a year, and a full mouth set of x-rays every 3-5 years.
